    To Volunteer at School we need : 

    1. Approved LPD(Livermore Police Department) background check - They do this by renewing or creating a Civicore account.  Can take up to 3 weeks, so should be done well in advance.  

    2. TB  - Tuberculosis test.  It's good for 4 yrs and comes from Providers(DR Office) Should be emailed to me at Jkraybill@lvjusd.org

    3.  Vaccine card - Email a picture of the card or brought in

    To Chaperone for a Field Trip we need:

    5.  Declaration page from your insurance showing that their insurance meets the District minimums outlined on the Transportation Document again emailed to Julie
